To help my patients budget for dental
treatment costs without compromising the
quality of care I am pleased to offer my own
Dental Plan which I believe to be the best
way to make treatment affordable and give
my patients greater freedom of choice.

Plan for your confidence
My Dental Plan enables you to spread the
cost of your family’s essential dental care.
This tailormade scheme has been carefully
designed to meet the needs of my patients
and to reflect my preventive philosophy, it
provides:
- Two check ups per year
- All necessary x-rays
- Cleaning and polishing – hygiene
appointments twice a year
- Oral hygiene and dietary instruction
- Routine fillings (non white)
- Access to our dental emergency service

of any additional treatments
Childrens plan
15% Discount on private fees
The investment of the adult Maintenance
Plan is just £14.95 per month.
Special plans for children -
protecting their dental
health
I also have a plan for children, which has
been designed to provide the foundation for
strong, healthy, adult teeth and gums. In
addition to the adult benefits listed above,
children also receive preventive treatment
and fissure sealants where necessary, plus
30% discount on private fees. Children
under 5 are free. For children who are 5-17
years old, it costs £7.50 per month and one
parent or guardian must be on a plan for a
child to be eligible. |

Family membership
The family discounts on the monthly
payments are:
2 people – 5%
3 people – 10%
4 people or more 15%
Please note that for family discounts all
patients must live at the same address and
use the same direct debit.
How to join the Plan
Simply fill out the direct debit and
application form and hand it to the
receptionist or post it to the address printed
on the back.
There is a one-off joining fee of £10 for
adults and £5 for children; this is taken with
the first monthly payment.
Your registration
Throughout your time as a patient of Dr
Acheampong, we appreciate that your
circumstances may change and so you may
withdraw from the scheme at any time.
Withdrawal will be effected 3 calendar
months after a written instruction is
received at the practice. If fees are paid
annually, upon withdrawal we will make a
refund proportional to the length of time
remaining to the end of the twelve-month
period. For patients paying monthly, direct
debit instalments collected after the
termination of the arrangement will be
refunded with the reduction of a £4
administration charge.

If you have any problems please contact
the Practice Manager who is our
complaints officer. Dr Acheampong and
his team will do their best to resolve
them. We have a practice complaints
procedure, which will deal with any
complaints about the Plan or any care, and
treatment we provide under it.
Emergency appointments
All patients who are members of the plan
are entitled to necessary care and
treatment in an emergency during normal
surgery hours.
